"Sooner or later, in the course of one's existence, each
one
we are confronted in spite of ourselves with tumors: four
people out of ten are affected directly and many
they have family members or acquaintances who have been diagnosed
an oncological pathology. Every year, in Switzerland, they
occur
38,000 new cases of cancer, a datum that tends
to grow mainly due to the progressive aging
of the population.
However, the number of new ones is not increasing
cases, but also that of people who, thanks to progress
in terms
of diagnosis and treatment, they survive
a tumor disease. Today, in our country, they live
about 300,000 people who are or have been ill
cancer. Twenty-five years ago they were only half.
It is a fact from which our society can not
regardless:
tumors are becoming a kind
of chronic pathology and this means, on the one hand, a lot of suffering
and, on the other hand, an important challenge in terms of
financial and human resources for the health system.
The Confederation and the
cantons reacted to
this situation by elaborating the National Strategy against
cancer 2014-2017, which establishes the main fields
intervention on prevention, assistance and
search. In the "Health2020" report, the Federal Council has inserted
the fight against cancer among health policy priorities.
The periodic processing of statistical data concerning
tumors is essential to improve the effectiveness of the
prevention, evaluate progress in the diagnostic field
and organize
health care appropriately.
This second report on cancer, published by the Office
Federal Statistical Office (FSO), by the National Institute for Epidemiology
and cancer registration (NICER) and from
Swiss Register of Pediatric Tumors (RSTP),
it is therefore
an important tool. To it will add well
soon the new federal law on the registration of the
cancer diseases, which provides for uniform registration
and complete data throughout Switzerland and improvement
of the possibilities
of international comparison.
Our knowledge about cancer is still limited.
For example, we do not know what causes development
of the disease, we are not always able to diagnose it
timely and the organization of assistance
coordinated
with patients remains complex. Hence the need
to continue to improve our data base
arrangement."
About 40 percent of people living in Switzerland are
they get sick of cancer in their life and diseases
tumors cause about 17,000 deaths
each year.
In the face of these findings, one can not but underline
the enormous political, health and social importance of the
national monitoring of tumors and related registration
full extent of cases in the whole of Switzerland. Officers
politicians, doctors and specialized organizations need

treatment.
Currently, in 23 cantons, they are registered in

the cancer registration system
in Switzerland.
The bill provides for the completion of the
epidemiological data provided today by the registries of tumors with
further information on the course of the disease and treatment
administered. In this way, the data of the registers
of the
tumors will be used in the future to ensure
quality in the oncology field and will be thrown solid
bases to optimize anticancer therapies and improve the
quality of life of the people involved.
This report is based on the data
of the cantonal registers
of tumors, of the Federal Statistical Office (FSO) e
of the Swiss Register of Pediatric Tumors (RSTP). In this
I intend to thank you as the president of NICER
all for the precious collaboration. The next
stage
will consist in bringing the results together in the Strategy
national cancer campaign 2014-2017, to which the NICER
is contributing in close cooperation with its own
partners at national level.
Tumors can occur at any age and affect newborns
as well as very old people. Although
infrequent, childhood neoplasms register each
around 190 new cases in Switzerland. Although the majority
part of the small patients can be cured, the deaths
however, for cancer they represent the second
most frequent
cause of mortality for this age group.
Pediatric tumors differ from those
of adulthood not only in terms of incidence, but
also and above all of localization, course of the
disease and response to therapies. We must also
keep
present that the children's organism is still undergoing
development and growth and reacts particularly
sensitive to certain forms of therapy. The disease and the
treatment are then often a source of great discomfort,
uncertainty
and tension and represent a huge challenge
for the family and the social circle of the sick child.
Assistance and support are therefore indispensable
motivated professionals from different disciplines.
Only the detection on the entire national
territory of
epidemiological data such as age, type of tumor, treatment
administered, course of the disease, place of residence
and many other important information can allow you to
draw precise conclusions on frequency and probability
healing of specific forms of cancer. Possible
causes, ie any external harmful factors, and the
long-term consequences of the disease and treatment
they must be able to be studied and analyzed also
years away, to positively influence e
lasting the chances of onset and of care of these
pathologies. The Swiss Register of Pediatric Tumors
(RSTP), a national population-based registry
which collects data concerning childhood neoplasms in
Switzerland, notes all new cases,
documents the treatments
and presents long-term investigations. In this way, guarantees
an important contribution to registration
of the causes of cancer, prevention and improvement of the
treatments and avoids late consequences.
The

The number of tumors increases ...
In the period 2008-2012 they were diagnosed about
21,000 new cancers per year among men and 17,500 between
women. Compared to the five-year period 2003-2007, they are
therefore
verified 2000 more cases for each of the two
sexes. Approximately 42,000 new diagnoses are estimated for 2015
of cancer, 23,000 among men and 19,000 among them
women.
... as the population ages
The main reason for the increase in cancer
cases
can be identified in the demographic trend and,
more precisely, in the strong aging of the population.
Indeed, considering all types of cancer, between
1998 and 2012, the risk of incidence remained almost constant
unchanged;
considering the standardized rates, that is
rates corrected by the effects of demographic trends,
the increase does not exceed 1 percent among women, while
for men there is a decline of 4 percent.
Four types of cancer make up half
of
new cases
Among men, 53 percent of new registered cases
every year it consists of prostate tumors, of the lung
and from colorectal tumors. For women, however, in
51 percent of the cases are breast and lung cancers
and of colorectal
tumors. No other location
affects more than 7 percent of new cases of cancer.
This report contains exhaustive data on 22 types of
tumor and, for the first time, presents information on
more rare tumor forms.
The risk of incidence evolves
differently over time
depending on the type of tumor
Between 1998 and 2012, the incidence of new cases of melanoma
and thyroid cancer is further increased
for both sexes. The same happened with the tumor
of the lung in the woman.
On the other hand, incidence rates significantly decreased
of laryngeal and stomach tumors as well as
in women, of cervical cancer.
Decreasing mortality for most types
of tumor
In the period 1983-2012 the standardized mortality rates
by age they fell 27 percent for women
and 36 percent for men. From 1998 mortality
it is in decline especially for laryngeal tumors, of the
stomach,
of the cervix, the breast, the prostate, i
colorectal tumors and non-Hodgkin's lymphoma.
The rate of
mortality related to lung cancer is decreasing
only for men, while for women it continues to
increase markedly.
About 16,000 people die of cancer every year
In Switzerland, cancer causes 9,000 deaths each year
between
men and 7,000 among women; 30 percent of the
deaths in men and 23 percent of those in women
they are due to the tumors. For men, 22 percent of the
deaths due to cancer is due to lung cancer, the
15 percent to prostate cancer and 10 percent
at the colorectal tumor. For women, breast cancer
is responsible for 19 percent of cancer deaths,
15 percent lung cancer and cancer
colorectal
10 percent. Overall, the tumor
of the lung is the form of cancer that causes the most
number of deaths (about 3000 per year).

The Ticino Cancer League supports and promotes scientific research in the oncology field through the Ticino Cancer Research Foundation.
The Ticino Foundation for Cancer Research was founded in
1984 thanks to an important private donation of about 1 million Swiss francs, with the agreement of the Ticino League against cancer which, by statute, holds the majority of seats on the board of foundation.
Since then, the Ticino Cancer League has delegated
to this Foundation the task of subsidizing cancer research in Italian Switzerland while guaranteeing funding, which varies from year to year.
Thanks to the important help of the Ticino League against cancer, the Foundation has been able to finance projects
for over 5 million francs for almost 30 years.
Which research is it?
Funding is aimed at projects ranging from the creation of a database on lymphogenous tumors (unique in Switzerland) to studies on the molecular mechanisms underlying the lymphomas
of the stomach, from the evaluation of the pharmacological behavior of many new drugs to the creation of models that allow us to design new types of anti-tumor therapies.
All these projects take place in collaboration with major academic centers and
funding is guaranteed only after a very severe evaluation by national and international experts.
Italian Switzerland can therefore make a qualified contribution to this huge global research movement which will one day finally overcome the various cancer
diseases.
All the enormous progress made in this field over the past 50 years has been the result of well-conducted and coordinated research. We can therefore say with confidence that this will happen in the future, especially if the necessary financial
resources are guaranteed.
